EffectiveSeptember 6, 2024, case counselors will submit the completed "Formal Case Preliminary Forms" packet (including the completed preliminary interview and the required documents) directly to the Tribunal. The Tribunal will then pair the case with a regional advocate who will assist with composing the libellus, and will place the case counselor and regional advocate in contact. It is no longer necessary for the case counselor to reach out to a regional advocate directly before submitting the case to the Tribunal.
These forms are only for use by trained case counselors and advocates. If you are interested in petitioning for an annulment, please do not attempt to fill out these forms on your own. Your parish can assist you in making an appointment with a trained case counselor to begin the annulment process.
