2017 Lumen Gentium Award Nomination Form

If honoree(s) serves in a ministry/organization that requires EIM compliance according to EIM policies, they must have an EIM application in the database that verifies current EIM compliance (a background check has been completed and approved and EIM workshop attendance is current within the past 3 years). Your EIM site administrator can verify the dates requested below. If honoree does not serve in a ministry that requires EIM compliance, the EIM office will use information provided above to run a background check on honoree(s).
